• Ervad Brigadier Panthaki and Mobedyar Zen Pandey will conduct the Muktad prayers
• Each family may submit up to 5 names for inclusion in the prayers
• Fill in the form and return to Ervad Brigadier Panthaki with a donation of
• $5.00 per name by August 1, 2014 (phone messages will not be accepted)
• Checks to be written in the name of ZAMWI
• Flowers may be offered at the ceremony
• You may bring a framed picture of the departed for the Muktad table
• Sandalwood and candles will be available on payment
• There will be a potluck dinner after the ceremony
